FORT WAYNE, Ind. — The PHP Foundation has awarded a $25,000 grant to Easterseals Northeast Indiana to support its On-Site Clinic for individuals with intellectual and developmental disabilities.

The funding will expand access to preventative medical and psychiatric care, helping reduce barriers to services and improve health outcomes. Individuals with disabilities often face challenges accessing healthcare, including transportation, communication, and finding providers experienced in meeting their unique needs. Easterseals’ On-Site Clinic helps eliminate those barriers by providing care in a familiar, accessible, and supportive environment.

“This funding helps ensure individuals can receive the care they need in a setting that is accessible, trusted, and tailored to them,” said Donna K. Elbrecht, CEO of Easterseals Northeast Indiana. “By focusing on prevention, early intervention, and coordinated care, we can support healthier lives, greater independence, and reduce the need for more intensive care.”
