About Adult Day Center

We offer adult day programs at all three of our locations with activities and outings oriented toward personal interests. Individuals who are more interested in preparing for employment and higher education may prefer our Pre-Employment Transition Services program.

Adult Day Center

Easterseals Northeast Indiana’s Adult Day Center in Fort Wayne provides a welcoming environment where adults age 18 and older with higher support needs can learn new skills, socialize with peers, and explore the community. Our evidence-based programs and caring staff ensure every participant has opportunities to thrive.

Participants can choose which groups or sessions to join, ensuring they remain engaged, motivated, and empowered to pursue activities they enjoy.

    We actively partner with local venues—including community centers, libraries, the YMCA, and more—to bring enriching experiences to our participants. Outings to festivals, seasonal events, plays, movies, the zoo, the conservatory, and other local attractions foster independence, social skills, and a strong sense of community connection.

  • Robust On-Site Programming
    Our daily calendar is packed with life-skills classes (like gardening, cooking), creative workshops (art, music, movement), and social groups, all led by community instructors and Easterseals staff. These offerings are designed to build confidence, promote independence, and celebrate every participant’s unique talents.

  • Themed Events & Celebrations
    From seasonal holidays to monthly themes, our center is always filled with festive decorations and lively events. These celebrations spark excitement, boost morale, and encourage meaningful participation all year long.

  • Nutritious Meals & Snacks
    We provide a hot, balanced lunch and an afternoon snack each day, ensuring participants are fueled and ready to make the most of their day. For those with specialized dietary requirements, we gladly accommodate by welcoming them to bring their own meals and snacks.

  • On-Site Health Clinic
    Operated by Rounding Providers, our on-site health clinic is available to all Adult Day Center attendees. This convenient resource ensures that health and wellness needs are addressed proactively and promptly, supporting overall well-being and peace of mind.


Quick Facts

Hours:
7:30 a.m. – 4:00 p.m., Monday through Friday
(Closed major holidays)

Participant-to-Staff Ratios:
Flexible group sizes up to 10:1; typically 6:1, 4:1, or 1:1 based on individual care plans.

Transportation:
Limited door-to-door transportation is available. We can also provide referrals to community transportation services.

Medical Care & Safety:
All staff are CPR- and first aid-certified and trained in medication administration. Individual health plans are strictly followed, and a nurse is on call for emergencies.

Accreditation:
CARF-accredited Adult Day Center, ensuring the highest quality standards of care and programming.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
