What Is Supported Living?

Supported Living offers one-to-four residents the opportunity to reside in a private house or apartment, creating a warm, home-like atmosphere. Dedicated Direct Support Professionals (DSPs) provide essential life-skills coaching and personalized support to empower each resident toward greater independence. This comprehensive assistance includes:

  • Medication management and administration, ensuring residents take their medications correctly and on time to maintain their health and well-being.

  • Personal grooming and hygiene, guiding residents through daily self-care routines that promote dignity and confidence.

  • Meal planning, cooking, and kitchen safety, teaching residents how to create nutritious, balanced meals while also practicing safe food handling and cooking techniques.

  • Home cleaning and maintenance, helping residents learn and apply housekeeping skills to keep their living spaces tidy and inviting.

  • Budgeting, bill payment, and financial oversight, assisting residents in managing their finances, paying bills on time, and making informed choices about spending and saving.

In addition, DSPs support community integration by encouraging participation in local activities, employment opportunities, and social events, fostering a sense of belonging and meaningful engagement with others. This holistic approach ensures each resident receives the support they need to thrive in their own home, with the comfort and security that comes from individualized care and attention.

Our team goes beyond daily routines by creating enriching experiences that align with each participant’s interests:

  • Local community events and volunteer opportunities

  • Regional excursions and day trips

  • Out-of-state travel adventures


Eligibility Requirements

To qualify for Supported Living:

  1. Hold a valid Community Integration and Habilitation (CIH) waiver.

  2. Demonstrate a need for support in areas such as health, safety, daily living skills, transportation, medication, and financial management.


24/7 Support & Call Center

Life doesn’t keep business hours—and neither do we. Our fully staffed call center is available 24 hours a day, 7 days a week at 260-969-4254 for:

  • Medication inquiries

  • Transportation coordination

  • Schedule updates

  • Emergency and non-emergency questions


Representative Payee Program

For individuals needing assistance with financial management, our Representative Payee Program ensures:

  • Timely bill payment

  • Monthly budgeting guidance

  • Transparent financial reporting to families and guardians


Safe, Reliable Transportation

Our DSPs use personal vehicles for daily transportation, with:

  • Annual driving-record checks and continuous proof of auto insurance

  • Agency-provided lift vans when specialized vehicles are required


Highly Trained, Certified Staff

Every member of our team is thoroughly trained to support your unique needs:

    • CPR & First Aid certified

    • Medication dispensing protocols

    • Individualized training on each participant’s support plan
