Preparing for More Independence

While other organizations may consider transition-age individuals to be those leaving high school, Easterseals Arc believes that individuals can achieve greater independence at any age. Therefore, our Transitions program serves any individual 18 or older who is unsure of their career or education pathway.

About Transitions

Transitions is a day program that caters to individual goals for independence. It has a heavy focus on community inclusion and preparing for community employment or post-secondary education.

The Transitions program focuses on:

  • Independent living skills such as budgeting, cooking, and hygiene. This is ideal for individuals who need to gain confidence before choosing to prepare for employment.
  • Work-based learning using volunteer opportunities, and employment outings to help participants develop foundational skills for employment and clarify their employment goals.
  • Community employment to help participants learn skills to gain employment in their desired field and explore the application process, interview preparation and self-advocacy. They also will have guidance in applying for Vocational Rehabilitiation Services as the next step in their employment journey.

Easterseals RISE

Our Transitions program in Angola is based at Cahoots Coffee Cafe, creating a unique opportunity for training in various employment oppportunities at the cafe. The Cahoots site also serves as a hub for Transitions participants to be picked up and dropped off for outings, work experiences and other activities.

Easterseals RISE also offers a Transitions program at its Auburn location.

More about Transitions

  • Daily activities are developed to address specific participant goals for steps toward community employment and increased independence.
  • Transitions participants can use Medicaid Waiver funding or private pay.
  • All staff are CPR-certified and trained on first aid and dispensing medication. Staff also are trained on each individual’s specific information.
